In order to affiliate with Dazzled, you have to follow a few rules.

.oo1 - Our banner must be up in the same section that you are requesting yours to be in.
.oo2 - You must use the form.
.oo3 - You must provide a 88x31 minibanner, no larger or smaller.
.oo4 - Your site must be active.

Now, here's our banner, so that you cn put it up on your site
